If LHO seems surprised to learn that he's not playing the hand, I'll lead a spade - he may have been expecting that his spade tenace would be protected.
Otherwise, I think a trump is right. In this sort of auction it's quite common for declarer to have a gap in his side suit, and dummy could well be 2-2 in the red suits.

Well analyzed by Gonzalo and Andy. In this case, declarer even had three trumps but he requires two ♥ ruffs after exiting to the ♥K. On any lead but a trump, declarer concedes a ♥ and ruffs the 4th ♥ with the beer card and you cannot overruff. 14 IMPs if you got this one right.
