Assign the blame given that East does not open his 11 count.

You can't get everything right. At least it went off!
While I might not agree with every call found at the table, I'd be looking elsewhere to find more matchpoints.
If someone has to have the blame, it would be East, who might bid 5H particularly at imps, but both one off is a live possibility.
North (and South) both found an action that many wouldn't.
