Using generic 2/1, what would 4 over 3NT mean here?

I don't think there is really a "generic 2/1" approach here - in most cases you just pass 3NT even with a 6 card suit. 3NT overcalls could be wide ranging and not necessarily a balanced hand, so any move should really be showing some form of slam interest rather than just improving the contract. 4 = hearts and 4 = spades so partner can superaccept is one option. 4 = something artificial also works.

I agree that South African transfers would make sense here.
Our inefficient but low memory load agreement is that over any natural 3NT with no previous bidding, 2NT-like systems are "on" where possible. So 4=Gerber unassigned, 4/4=major transfer, 4=minors, 4NT = quantitative.
