If we are not going to pass 3NT over 3♠, why 3♠ ? 4♠ as splinter would be much better bid imo if we are not going to pass 3NT.

4S takes up a lot of room. After 3S-3N you can cuebid. It seems like your hand is too strong to just bid 4S and pass 5D. Partner will probably not like his hand with Kxx xx QJxxx Kxx or something but that is a good slam. If partner has a spade stopper and a heart cuebid, slam is less likely to be good though and you can find out.

I splintered. Partner held ATx QTx Qxxx 7xx. This is a pretty lousy slam - partner felt boxed over 4♠ I think and keycarded us into 6.
For a second it looked reasonable. Low club lead ducked (!) and K on right. But trump were 4-1 and the heart finesse lost.

3S definitely then over 3NT either pass or bid 4D, depending on how lucky you feel. Probably pass, since the spade wastage is a bad omen.
Definitely drive to slam if pard doesn't bid 3NT.
