This is a recurring theme but I have rarely seen it so extreme. I wonder what went wrong. 4♦ meant as a some kind of COG but taken as a sign off? Could it really be a simulation fluke?

I think the real problem here is the pass of partner's 3NT is not defined. It has to be limited so that pulling later doesn't show a good hand. Then west wouldn't bid, 4 card suit or not.

I think your opinion is not a realistic imagination.
First see Helene_t hand.
Second see my basic Gib hand.

As we know rebid-4♣ after opp's double usually shows weakish hand, however you have no way to limit TP evaluation.
Now see Helene_t hand, 3♣ has promised 14+hcp, when west really holds 11+hcp, they have no way to sign off at four level for ever and ever.
This be a real problem in fact.
TP is not a good evaluation.
